Please inform the admissions office of any change in your address or personal details (telephone, e-mail).

If you contact the centre, either by mail or by phone, please provide your medical record reference, which you will find on your appointment card.

The postal and electronic mail addresses you provide to the centre may be used to contact you for the purposes of surveys.
