Each time you visit, ask the receptionist in the lobby for directions.
Creating your administrative file
Your administrative file will be created at theAdmissions Office, located in the Center's entrance hall. The office is open from 8 a.m. to 5 p.m., Monday through Friday.
Outside of the Admissions Office's opening hours, please contact the "Switchboard – Security PC," also located in the entrance hall.
To create your file, the agent at the admissions office will ask you for the following documents:
- official identification (national identity card, passport),
- your carte vitale,
- your valid health insurance card.
PIN
The admissions office will give you an appointment card containing your personal identification number (PIN). You will be asked for this appointment card and your PIN every time you come to the Centre or call.
The Identity Vigilance Charter
The regional identitovigilance charter requires healthcare professionals to work on the basis of patients' birth names.
To ensure safety and quality of care, everyone needs to "make sure they are properly identified by their caregivers" throughout their treatment.
Your identification is the first essential and fundamental step
before any act of care.
That's why :
- You will be asked to show proof of identity when you create your file,
- an identification bracelet is worn during hospitalization,
- We frequently ask for your first and last name and date of birth , at every stage of your treatment.
Don't hesitate to identify yourself each time you receive a new treatment.

For each hospitalization or consultation
Don't forget to bring any X-rays or medical test results you may have, to give to the nursing staff or medical assistant.
Bring along your current treatment prescriptions. These documents will be scanned into your patient file and returned to you.
Have you been here before?
The admission procedure is straightforward, and will be explained to you by the department staff who will be taking charge of you as part of your care plan.
Don't forget to include any new information that has arisen since your last visit (prescription, hospital and/or radiology report, change of marital status, change of contact person, change of address, etc.).
ANONYMAT
You can ask the admissions office not to disclose your presence on the premises, or to receive telephone calls.
YOURCOORDINATES
Notify the secretariat of the department in charge of you of any change of address or contact details (telephone, e-mail). The postal and e-mail addresses you provide may be used by the Center to contact you for survey purposes.
