Preparation for your discharge is carried out in close collaboration with you, your family or caregivers. Professionals take into account your environmental and social living conditions, in order to best organize your discharge.
A discharge plan is proposed to you. It is designed to enable you to continue your care beyond the Center.

Discharge liaison letter

It is given to you in person and sent to your attending physician (unless you object). It summarizes your care during your stay.
A hospitalization report is sent to your attending physician.

Exit formalities

They are carried out in your hospital ward. The unit nurse will give you all the documents you need to monitor your care.

At your request, a status report can be given to you on discharge by the department in which you were hospitalized. It serves as a work stoppage for the duration of your hospitalization.

Payment

If you do not have a health insurance plan, or if your health insurance plan has not been approved by the Centre François Baclesse, you must pay at the admissions office:

  • of the daily rate,
  • of the co-payment for your hospital stay.

Continuity of care

If your care needs to be continued at home or in another facility, dedicated teams will provide you with their expertise.
They will organize your care according to your situation, in collaboration with independent healthcare professionals, service providers, care networks or via a home hospitalization service (HAD).

Home Healthcare Providers

Home Health Care Providers (HHCPs ) are teams of healthcare professionals who implement logistical, technical and technological solutions to help patients return to or remain at home. In the care of oncology patients, they are full-fledged players in the continuity of care, being one of the links in our healthcare territory.
In particular, they liaise with all the healthcare professionals involved in the care pathway (general practitioner, private nurses, hospital doctor, etc.).).
At the Centre Francois Baclesse, we propose that PSADs make a commitment to our teams through a Charter of Good Practice, in order to offer high-quality care adapted to the specific needs of oncology.

City pharmacies are not covered by this approach.
Finally, patients are free to choose the Home Health Care Provider of their choice, whether or not it is one of the Health Care Providers who have signed the Charter.

In order to improve the quality of services and care offered to patients, our facility is an active participant in the " e-Satis " national initiative to measure the satisfaction of hospitalized patients.

At the end of your hospital stay, you will be invited by e-mail to give your point of view on the quality of your care via a totally anonymous online questionnaire.

You also have the option of completing a paper satisfaction questionnaire, which the care team will suggest to you.
